MOOD & EMOTION TRACKER Daily Mood Monitoring, Emotion Awareness & Weekly Review
Section 1: Depression Screening — PHQ-9
The PHQ-9 (Kroenke & Spitzer, 2002) is a validated 9-item depression screening tool. Complete at the start of treatment and every 2–4 weeks.
Section 2: Daily Mood Log
Complete once daily — ideally in the evening or at a consistent time. Rate overall mood and any notable emotions. Brief notes about what influenced your mood are very helpful.
Section 3: Emotion Identification
Research shows that the ability to precisely name emotions — called 'emotional granularity' or 'affect labeling' — is linked to better emotional regulation and mental health outcomes (Torre & Lieberman, 2018; Kashdan et al., 2015).
Section 4: Weekly Mood Review
Review your daily mood log and answer these questions each week. This builds self-awareness and helps you identify patterns.
Section 5: Monthly Mood Overview
Section 5: Monthly Mood Overview
